Below is information about signing up to participate in Woodville Jaguars Pop Warner Programs.
What are the Requirements to Participate in Pop Warner?
- Must be between 5-15 years of age as of July 31st.
- Participant must have a 70%/2.0 grade point average from the previous school year
Those not meeting the scholastic requirement may submit a Scholastic Eligibility Form (if applicable), a progress report system used to monitor the participants grades throughout the season.
What Do I Need to Bring to Registration?
The following forms may be printed and signed by hand. Electronic signatures will now be accepted.
- Original Birth Certificate (with raised seal).
- Proof of Residency
- 2023 Player/Participant Contract - Please upload to participate registration portal.
- 2023 Medical/Physical Form - signed and stamped by physician.
- Year-End report card for current year (4 quarters).*Please upload to participants registration portal*
- 2023 Woodville Football Equipment Policy or 2023 Woodville Cheer Equipment Policy
- Registration Cost is $125.00 until August 1st. Sibling discounts available.
No participant will be permitted to participate on the practice field or on game days without all needed forms completed and submitted.
